Does ForLife take insurance?
No, ForLife Physical Therapy and Performance is a cash-based practice and does not accept insurance. This model allows us to provide a higher level of care than what is typically covered by insurance- based clinics. Instead of being restricted by insurance limitations–such as visit caps, rushed appointments, generalized treatment plans, and restrictions in the type of care you receive– we focus on personalized, one-on-one care tailored specifically to your needs.
Our goal is to prioritize your long-term health and performance goals without the constraints of insurance companies determining what is “medically necessary.” This means longer, more effective sessions, faster progress, and ability to reach higher level goals.

What makes ForLife different than traditional physical therapy or personal training?
ForLife Physical Therapy and Performance is designed to give you a level of care and coaching that goes beyond what’s typically found in insurance based physical therapy clinics or standard personal training programs.
Our goal is to bridge the gap between rehab and performance. Whether you’re an athlete returning to sport, an active individual looking to prevent injury, or someone who wants to move and feel better in daily life, we provide a comprehensive approach that ensures long-term results, not just temporary fixes.

Do I need a referral?
You are not required to have a referral from a doctor but if you have one you are welcome to bring it.

What should I bring to my first session?
Just wear comfortable clothing that allows you to move freely!
If you have any relevant medical records, imaging reports, or past training programs you’d like to discuss, you're welcome to bring them, but they’re not required.
